[摘 要] 目的 评价窄谱UVB治疗寻常性银屑病的疗效及安全性,并与改良
PUVA(PUVA+UVB)疗法比较。
方法 108例患者,其中50例单纯照射窄谱UVB;58例口服8-甲氧补骨脂素片,0.6
mg/kg/次 ,2 h后照射UVA和UVB;每周3次,疗程8周,观察疗效与安全性。
结果 经过8周治疗,窄谱UVB组基愈率为70.0%,显效率为
20.0%。改良PUVA组基愈率为96.6%,显效率为3.4%;不良反应:窄谱UVB组仅个别患者出现轻度红斑、色素沉着及皮肤瘙痒。改良PUVA组发生率为40.3%,多为轻中度消化道反应及皮肤瘙痒、色素沉着等。照射时间窄谱UVB组最长不超过7
min,改良PUVA组最长需30 min以上。
结论 窄谱UVB治疗寻常性斑块型银屑病具有良好疗效,与改良PUVA疗法相比,虽然疗效略逊,但方法简便,患者无需服药,照光时间短,不良反应发生率亦低。

Abstract:
Objective
To evaluate the efficacy and safety of NB-UVB,
in comparison with modified PUVA(PUVA+UVB) in patients with vulgar
psoriasis.
Methods
108 patients affected by vulgar chronic plaque psoriasis,of
which 50 subjects were only radiated with narrowband
UVB,and 58 subjects were orally administered with
8-MOP,0.6 mg/kg body
weight.2 hours later radiated with UVA and UVB. All subjects were radiated 3
times per week,with 8week
treatment duration.
Results
After 8-week treatment course,the
basically effective rate in narrowband group was
70%, and the significantly effective rate was
20%.Where in the modified PUVA group,the basically
effective rate was 96.6%,and the significantly
effective rate was 3.4%.Adverse drug reactions:only some subjects in narrowband
group experienced mild erythema,pigmentation and
pruititis. But adverse drug reaction rate in modified PUVA group was 40.3%,most
of them were mild or moderate gastrointestinal reactions,pruititis
and pigmentation etc.Radiation duration.The longest time was not longer than
7 minutes for narrowband group,but
it was up to 30 minutes for modified PUVA group subjects.
Conclusion
Narrowband UVB has favorable
effect for vulgar chronic plaque psoriasis.Though narrowband
UVB is a little less effective than modified PUVA method,it
is simple to conduct,having no need to take drugs,with
short radiation time and lower incidence of adverse reactions.
