[摘
要] 目的
阐明多点接种真菌培养法在实验室诊断甲真菌病的作用和意义。
方法
采用多点接种真菌培养法和传统的点植培养法对两组甲真菌病患者的甲屑进行致病真菌的分离和鉴定,比较两种方法检出甲真菌病致病真菌的构成比例。
结果
试验组和对照组培养阳性率分别为为53.04%和37.50%(P=0.029)。致病菌分离鉴定结果:试验组皮肤癣菌49.21%,酵母菌36.51%,霉菌14.29%;对照组则分别为76.92%,20.51%和2.82%(P
=0.001)。试验组共检出2例混合感染,对照组则无。
结论
多点接种法真菌培养可以提高对甲真菌病的病原菌的检出率,尤其是对非皮肤癣菌酵母菌和霉菌的检出率,且有助于发现甲真菌病的混合感染,但对非皮肤癣菌致病性的判定有待进一步研究。

Abstract:
Objective To study the role of Multiple
Spots Inoculation of Fungi in Sabouraud agar for the diagnosis of
Onychomycosis,especially evaluating pathogenic
potential of nondermatophytes and differentiating
the mixed infection.
Methods
Pathogen of onychomycosis was detected by multiple
spots inoculation fungal culture and single spot inoculation.,and
the isolated fungi results of the study group and the control group should
be compared.
Results
The positive rate was 53.04% in the study group and
37.50% in the control group.(P=0.029).
The isolated fungi results of the study group were dermatophytes 49.21%,yeasts
36.51% and moulds14.29%,and the of the control
group were 76.92%,20.51% and 2.82% (P=0.001).
Totally 2 mixed infection were identified from study group.
Conclusion
Multiple spots inoculation fungal culture could
increase the fungal detection rate.and increase the non-dermatophyte and
mould detection rate,and was helpful on detection
and differentiation of the mixed infection of the onychomycosis,but
more studies should be done on the criterion for the judgement of pathogenic
potential of nondermatophytes.
